- Epinephrine 1:1000 0.5ml IM every 5 minutes (.01ml/kg)
- Respiratory support:
- Oxygen 6L/mt by FM or NC
- Intubate if needed
- Albuterol 0.5% 0.5ml in 2.5ml NS q 30 minutes by Nebulizer
- H1 blocker: Diphenhydramine (Benadryl) 1mg/kg IV or IM (max 50 mg) DO NOT GIVE BEFORE EPINEPHRINE
- H2 Blocker: Ranitidine 1mg/kg IV
- Steroids: Solumedrol 250mg IV once followed by 125 mg IV q6h
- Inotropic support:
- Dopamine 2-20 microgram/kg/minute IV, titrate to obtain SBP >100
- OR
- Levophed (norepinephrine 8-12 micrograms/minute IV , titrate to obtain SBP >100
