Acute Stroke in Atrial Fibrillation , non-TPA candidates:
- May start patient on Aspirin instead of Heparin
- IV Heparin is a consideration , but avoid if the stroke is large
- SQ Heparin for DVT prophylaxis if significant leg weakness present
- Initiate Warfarin 3 days after the event, after a repeat CT scan
- Aim to make INR therapeutic by 7th-10th day
- Delay warfarin if significant hemorrhage is present
